The application must be filled out completely. Missing information will delay application processing as well as disbursement of funds.

Completing this application provides no guarantee that you will receive funds from HeartStone Foundation.

Once the board receives a complete application, you will be notified that it was received. The review process takes approximately 14 days and at that point, a board member will contact you.

The HeartStone Foundation will not name you as a recipient publicly without your written consent. We do hope you will fill out the release form that is included in your application. We want to share your story to help raise more funds for others in need. Help us pay it forward!
